Your student WILL NOT be able to start school on August 19, 2024, without the Meningococcal vaccine.
The Prince William Health District will be at Hylton High School on April 2, 2024, between 8:30 a.m. and 1 p.m. to administer the meningococcal vaccine for free. If you would like your 11th grade student to receive this free vaccine, complete the required parental consent form below. The completed parental consent form must be returned to the school health clinic on or before April 2, 2024. Vaccines are provided on a first come first serve basis while supplies last.
You may also obtain a copy of the required parental consent form in the front office at Hylton High School, in the school health clinic, or on the school’s Immunization Clinic webpage.
If your student has already received both meningococcal vaccines, please provide proof to your student’s school nurse. You may view information about your student’s compliance with required immunizations in ParentVUE. If you do not have a ParentVUE account, please contact your student’s school.
